Based on the criteria set forth below, twelve points are needed to be eligible for this subject matter panel. Please fill in the number of points that you have acquired for each criteria.

CRITERIA	# OF POINTS
One point for each credit hour of taxation CLE obtained within the past two years, maximum four points.
One point for attending a Taxation Committee meetings of a local, state, or national bar association or organization attended within the past two years, maximum three points.
Two points for each case where attorney is counsel of record for the taxpayer in the past two years, with case proceeding through examination level with IRS or through audit and initial administrative appeal level with state revenue department (i.e. Ohio Department of Taxation) maximum six points.
Six points for each taxation case where attorney is counsel of record for taxpayer with case proceeding through office of appeals of IRS or Appellate Division/Board of Tax Appeals of state revenue department, maximum twelve points.
Six points (maximum) for teaching a taxation law course for CLE credit at either the local or national level within the past year.
One point (maximum) for having published an article pertaining to taxation law within the past five years.
Twelve points if attorney is a currently licensed CPA.
Twelve points for current certification as a Federal Taxation specialist by the Ohio Supreme Court Commission.
