Based on the criteria set forth below, twelve points are needed to be eligible for this subject matter panel. Please fill in the number of points that you have acquired for each criteria.

CRITERIA	# OF POINTS
One point for each credit hour of CLE related to Sports Law and obtained within the past two years, maximum four points.
One point for attending a Sports and Entertainment Law Committee meetings of a local, state, or national bar association or organization within the past two years, maximum three points.
Four points for representation of a client in a transaction or dispute, or for an appearance as counsel of record; or “of counsel” in litigation involving Sports Law, maximum twelve points.
Six points (maximum) for teaching a sports law course for CLE credit at either the local or national level within the past year.
One point (maximum) for having published an article pertaining to sports law within the past five years.
