Based on the criteria set forth below, twelve points are needed to be eligible for this subject matter panel. Please fill in the number of points that you have acquired for each criteria.

CRITERIA	# OF POINTS
NOSSCR or CLE attendance within the past two years, one point per CLE credit hour, maximum four points
One point for attending a Social Security Committee meetings of a local, state, or national bar association or organization within the past two years, maximum three points.
One point for each case for representation of a claimant at the ALJ level, either to the point of an on the record decision following the ALJ hearing, maximum six points.
One point per appeal before the Social Security Appeals Council, maximum three points.
One point for each social security claim represented, maximum three points.
One point per claim for representation of an individual at the Reconsideration stage, maximum three points.
Three points for each year of employment at the Social Security Administration within the last two years, maximum six points.
One point for each claim where attorney has clerked with a law firm and assisted on a Social Security claim within the past five years, maximum three points.
One point (maximum) for having published an article pertaining to social security law within the past five years.
Six points (maximum) for teaching a Social Security Benefits course for CLE credit at either the local or national level within the past year.
Twelve points for current certification as a social security disability specialist by the Ohio Supreme Court Commission.
