Based on the criteria set forth below, twelve points are needed to be eligible for this subject matter panel. Please fill in the number of points that you have acquired for each criteria.

CRITERIA	# OF POINTS
One point for attending a meeting of the following organizations: Tort and Insurance Committee of any local, state, or national Bar Association, Hamilton County Trial Lawyers, Ohio Academy of Trial Lawyers, American Trial Lawyers’ Association (ALTA) within the past two years, maximum three points.
One point per credit hour of CLE taken in Alternative Dispute Resolution, Torts, Trial Skills/ Advocacy or Discovery Proceedings within the past two years, maximum four points.
Six points (maximum) for teaching a tort course for CLE credit at either the local or national level within the past year.
One point (maximum) for having published an article pertaining to torts within the past five years.
One point for settling or handling a tort case before any pleadings were filed within the past five years. One additional point for settling or handling a tort case after any pleadings were filed within eth past five years. One additional point for participating in Alternative Dispute Resolution pertaining to a tort case within the past five years. Two additional points for each tort case taken to bench trial within the past five years. Six additional points for each tort case taken to jury trial within the past five years. Maximum twelve points.
