Based on the criteria set forth below, twelve points are needed to be eligible for this subject matter panel. Please fill in the number of points that you have acquired for each criteria.

CRITERIA	# OF POINTS
One point for each credit hour of CLE in Employee Benefits obtained within the past two years, maximum four points.
One point for attending an Employee Benefits Committee meeting of a local, state or national professional association/organization within the past two years, maximum three points.
One point for each client represented in an employment benefits case: Two additional points if case is filed for summary judgement Six points for client represented in bench trial Twelve points for client represented in jury trial Maximum twelve points.
Six points (maximum) for teaching an employment benefits law course for CLE credit at either the local or national level within the past year.
One point (maximum) for having published an article pertaining to employment benefits law within the past five years.
