Based on the criteria set forth below, twelve points are needed to be eligible for this subject matter panel. Please fill in the number of points that you have acquired for each criteria.

CRITERIA	# OF POINTS
One point for each credit hour of CLE taken in a topic related to collections within the past two years, maximum four points.
One point for attending a Financial Law Committee meetings of a local, state, or national bar association within the past two years, maximum three points.
One point for each client counseled on a collections issue, maximum six points.
Two points for each collections case handled in Municipal or Common Pleas Court within the past five years An additional two points for each case where the attorney has either prepared or defended against a motion for summary judgement or negotiated a settlement after the case was filed An additional four points for each case taken to bench trial An additional six points for each case where the attorney has taken the case through a jury trial Maximum twelve points.
Six points (maximum) for teaching a collections law course for CLE credit at either the local or national level within the past year.
One point (maximum) for having published an article pertaining to collections law within the past five years.
Twelve points for current certification as a Creditor’s Rights/Debt Collection specialist by the Ohio Supreme Court Commission.
